WHEREAS, pursuant to Resolution 18-001, the Board of County Commissioners (BOCC)
adopted the amended 2018-2023 Transportation Improvement Program (TIP), and;
WHEREAS, these projects are listed on Lewis County's amended 2018-2023 TIP, and;
WHEREAS, this contract provides for the improvement of Bunker Creek and Pleasant Valley
roads in Lewis County by purchasing culverts for future installation and other related work, all in
accordance with the Contract Plans, Contract Provisions, and the Standard Specifications, and;
WHEREAS, pursuant to Resolution 18-116, the BOCC adopted the 2018 Standard
Specifications for Road, Bridge, and Municipal Construction and this proposal shall be performed by
contract in accordance with these specifications.
NOW THEREFORE BE IT RESOLVED that the Department of Public Works is instructed to
issue a Call for Bids and Contract Specifications for the material purchase of two concrete box
culverts (CMP 1603 and 1604). Bids for this proposal shall be submitted to the Clerk of the BOCC
(351 N.W. North Street, Room 210, CMS-01, Chehalis, Washington 98532) by or before 11:00
a.m. on Tuesday, April 24, 2018 and will be opened on or after 11:00 a.m. on April 24, 2018,
subject to official proposal addenda that establishes an alternate bid due date or opening time.

CALL FOR BIDS
NOTICE IS HEREBY GIVEN that the Board of County Commissioners of Lewis
County or designee, will open sealed proposals and publicly read them aloud on or after 11:00 a.m. on
Tuesday, April 24, 2018, at the Lewis County Courthouse, Chehalis, Washington, for the material
purchase of two precast concrete split box culverts (CMP 1603 and 1604).This contract provides
for the improvement of*** Pleasant Valley and Bunker Creek Roads by purchasing precast
concrete split box culverts for future installation, *** and other work, all in accordance with
these Contract Provisions, and the Standard Specifications.
SEALED BIDS MUST BE DELIVERED BY OR BEFORE
11:00 A.M. on Tuesday, April 24, 2018
(Lewis County official time is displayed on Axxess Intertel phones in the office of the Board of County Commissioners. Bids
submitted after 11:00 AM will not be considered for this project.)
Sealed proposals must be delivered to the Clerk of the Board of Lewis County Commissioners
(351 N.W. North Street, Room 210, CMS-01, Chehalis, Washington 98532), by or before 11:00 A.M. on the
date specified for opening, and in an envelope clearly marked: "SEALED BID FOR THE MATERIAL
PURCHASE OF TWO PRECAST CONCRETE SPLIT BOX CULVERTS (CMP 1603 AND 1604), TO BE
OPENED ON OR AFTER 11:00 A.M. ON April 24, 2018.
All bid proposals shall be accompanied by a bid proposal deposit in cash, certified check, cashier's
check or surety bond in an amount equal to five percent (5%) of the amount of such bid proposal.
Should the successful bidder fail to enter into such contract and furnish satisfactory performance
bond within the time stated in the specifications, the bid proposal deposit shall be forfeited to the
Lewis County Public Works Department.
Informational copies of maps, plans and specifications are on file for inspection in the office of the
County Engineer of Lewis County in Chehalis, Washington. The contract documents may be
viewed and downloaded from Lewis County's Web Site @ www.lewiscountywa.gov/or you may call
the Lewis County Engineers office @ (360)740-2612 and request a copy be mailed to you. All
Contractor questions and Lewis County clarifying answers will be posted on our website and
emailed to all Contractors registered on Lewis County's Planholder List. Plan or specification
changes shall be accomplished through official project addendums.
The Lewis County Public Works Department in accordance with Title VI of the Civil Rights Act of
1964, 78 Stat. 252, 42 U.S.C. 2000d to 2000d-4 and Title 49, Code of Federal Regulations,
Department of Transportation, subtitle A, Office of the Secretary, Part 21, nondiscrimination in
Federally assisted programs of the Department of Transportation issued pursuant to such Act,
hereby notifies all bidders that it will affirmatively insure that in any contract entered into pursuant
to this advertisement, disadvantaged business enterprises as defined at 49 CFR Part 26 will be
afforded full opportunity to submit bids in response to this invitation and will not be discriminated
against on the grounds of race, color, or national origin, or sex in consideration for an award.
The BOCC reserves the right to reject any or all bids and to waive all informalities in the bidding.

Description
This contract provides for the production, delivery and stockpiling of two precast concrete split-box culverts
with wingwalls. The box culvert structures shall support an AASHTO HL-93 loading with a maximum soil
bearing pressure of 3,000 pounds per square foot. Precast concrete split-box culverts include structures 18-ft
wide x 10-ft high x 84-ft long (Prairie Creek, CMP 1604) and 14-ft wide x 7-ft high x 52-ft long (Stearns
Creek Tributary, CMP 1603), both structures include wingwalls at all four corners. This material purchase will
allow Lewis County crews to complete the planned maintenance on Pleasant Valley Rd MP 2.179 and Bunker
Creek Rd MP 5.678. These projects are listed on Lewis County's previously adopted 2018 Six-Year
Transportation Improvement Program.
Sealed bids for the material purchase of two culverts shall be submitted to the Clerk of the Board of Lewis
County Commissioners (351 N.W. North Street, Room 210, CMS-01,Chehalis, Washington 98532), by or
before 11:00 a.m. on Tuesday, April 24, 2018 and will be opened on or after 11:00 a.m. on April 24,2018. Bids
submitted after 11:00 a.m. will not be considered for this proposal. This contract will be available for viewing
on Lewis County's website after the successful execution of the Bid Call.
