IN RE ACCEPTANCE OF AN AMENDMENT ) Resolution No 19- 051
BETWEEN THE LEWIS COUNTY SHERIFF )
AND NAPHCARE, INC )
WHEREAS the parties wish to add an Amendment to the Agreement, and
WHEREAS, the County has become a Medicaid Transformation partnenng provider pursuant to
Washington State's Medicaid Transformation Project and has been granted funding for a Medication
Assisted Treatment(MAT) program, and
WHEREAS, so long as funding for the program is available, the County desires for Contractor to
assist in providing staffing and services in relation to the program,
WHEREAS, it appears to be in the best public interest to authonze the execution of said
agreements
NOW THEREFORE BE IT RESOLVED that the Shenff or, in his absence, the Undershenff or
Corrections Chief, is authonzed to sign the subjoined Agreement on behalf of Lewis County

FOURTH AMENDMENT TO PERSONAL SERVICES AGREEMENT
This Agreement, made and entered into on the day of February, 2019, is effective
on 2019, is the Fourth Amendment to the Personal Services
Agreement made and entered into on the 1't day of February 2017 by and between NaphCare,
Inc ("Contractor") and Lewis County ("County"), as well as to all exhibits thereto
("Agreement")
WHEREAS, the County has become a Medicaid Transformation partnering provider
pursuant to Washington State's Medicaid Transformation Project and has been granted funding
for a Medication Assisted Treatment (MAT) program, and
WHEREAS, so long as funding for the program is available, the County desires for
Contractor to assist in providing staffing and services in relation to the program,
NOW THEREFORE, the Agreement shall remain in full force and effect with the
following addition and/or modification fully incorporated thereto
I So long as funding for the MAT program is available to the County, Contractor
shall provide a 6 FTE LPN position (24 hours LPN per week)
II So long as funding for the MAT program is available to the County, Contractor
shall be compensated $41,000 00 per year as of the effective date, for the
additional LPN hours Said amount shall be payable to Contractor on monthly
basis pursuant to Contractor's submitted monthly invoice
IN WITNESS WHEREOF, the parties have executed this Agreement as of the Effective
Date
