Cost Change amendment with RCO for Scammon Creek BEFORE THE BOARD OF COUNTY COMMISSIONERS OF LEWIS COUNTY, WASHINGTON IN THE MATTER OF } Approving a Cost Change Amendment between Lewis ) Resolution No 19-2 Zcl County and the Recreation and Conservation Office (RCO) for } Scammon Creek (RM 1 15) Barrier Removal/Graf Road MP 1 01 } WHEREAS, Public Works has listed fish passage restoration projects on the maintenance program and WHEREAS, Scammon Creek (RM 1 15) Barrier Removal/Graf Road MP 1 01, RCO #14- 1267R, CMP 1531 was awarded $184,240 00 in state WCRI funds through the RCO which was approved by Resolution No 15-377, and $90,000 00 in additional state WCRI funds through RCO which was approved by Resolution No 18-151, and WHEREAS, Public Works has secured additional funding for this project through the RCO in the amount of$561,560 00 in state FBRB funds, and WHEREAS the Public Works County Engineer has reviewed the Cost Change Amendment between Lewis County and RCO NOW, THEREFORE BE IT RESOLVED that the BOCC has determined that proceeding with the Scammon Creek (RM 1 15) Barrier Removal/Graf Road MP 1 01, RCO #14-1267R CMP 1531 to be in the best public interest and hereby approves the Cost Change Amendment, and the Chair of the BOCC is authorized to sign the same PASSED IN REGULAR SESSION this 12th day of August, 2019 APPROVED AS TO FORM BOA-D OF COUNTY CO MISSIONERS Jonathan L Meyer Prosecuting Attorney LE . These culverts are estimated to be 33% passable due to a calculated low flow depth of 0.13 m and an observed low flow depth of 0 03 m These culverts were ranked 4th for replacement within the Middle Chehalis Basin Watershed Culvert Assessment, dated December 2003, and are ranked 1st for replacement by Lewis County When completed the project goal will restore salmon access to an additional 10.98 linear miles of potential habitat 771 square meters (0 19 acres) of spawning habitat, and 18,868 square meters (4.66 acres) of rearing habitat for the Southwest Washington ESU of coho salmon, as well as searun cutthroat and resident cutthroat trout • AAGREEDSC.RPT July 17 2019 Page 1 Amendment Special Conditions Project Sponsor Lewis County Public Works Project Number 14-1267 R Project Title- Scammon Creek (RM 1 15) Barrier Removal Amendment Number 4 Special Conditions Cultural Resources Review This project is subject to the National Historic Preservation Act, Section 106, and therefore appears to be exempt from Governor's Executive Order 05-05 Archaeological and Cultural Resources (EO 05-05) as described in Section 9 of this project agreement In order for this project to be exempt from EO 05-05, the Section 106 Area of Potential Effect(APE) must include all ground-disturbing activities subject to this project agreement, including the restoration staging area The sponsor is encouraged to work with the federal permitting agency to align the Section 106 APE with the scope of work subject to this project agreement If the APE does not include all ground-disturbing activities subject to this project agreement, promptly notify the RCO grant manager as this will require RCO to initiate cultural resources consultation following EO 05-05 for those activities not included in the federal APE Completion of this consultation and a Notice to Proceed from RCO will be required before these ground-disturbing activities can begin Full Design and Construction Deliverables The sponsor will meet the standards described in 2015 Manual 18 Appendices D-2, D-3, and D-4 for design and construction The following must be submitted to RCO 1) Preliminary Design Deliverables(See Appendix D-2 for additional details) a) Preliminary Design Report, Drawings, and Engineering Cost Estimate, b) Design Review Comments (optional), and c) Permit Applications(optional) 2) Final Design Deliverables will be provided to RCO before construction (See Appendix D-3 for additional details) a) Design review comments, b) Final design report and drawings incorporating comments provided by stakeholders SRFB and/or permit agencies regarding the preliminary design report and on-site review c) Technical specifications (Do not need to be provided in PRISM if documentation is large Sponsor should maintain in office files following required documentation retention requirements for SRFB funded projects) d) Final construction quantities and costs e) Contract bidding documents and general contract conditions (Do not need to be provided in PRISM if documentation is large Sponsor should maintain in office files following required documentation retention requirements for SRFB funded projects) f) Permit applications and permits including cultural and historic resources review (prior to beginning construction activities) g) Control and tenure documentation (prior to beginning construction activities) 3) Post-Construction Deliverables (See Appendix D-4 for additional details) a) "As-Built' Drawings after construction is complete ASPECCOND.RPT July 17 2019 Page 1 Amendment Eligible Scope Activities Project Sponsor.
