Approve Amendment D to contract with The Salvation Army BEFORE THE BOARD OF COUNTY COMMISSIONERS
LEWIS COUNTY, WASHINGTON
IN THE MATTER OF: RESOLUTION NO 19-390
APPROVING AMENDMENT D TO CONTRACT
BETWEEN LEWIS COUNTY AND THE SALVATION
ARMY
WHEREAS, the Board of County Commissioners (BOCC) of Lewis County, Washington,
has reviewed contract amendment #T01-0011-19-D between Lewis County and The
Salvation Army for the period of January 1, 2018, through June 30, 2020, and
WHEREAS, additional funding of $60,000 increases the total amount to $199,042, and
WHEREAS, termination of the agreement is extended from December 31, 2019, to June
30, 2020, and
WHEREAS, contracted services are for homeless prevention for persons at risk residing
in Lewis County, and
WHEREAS, it appears to be in the best public interest to authorize the execution of
said contract for Lewis County
NOW THEREFORE BE IT RESOLVED that contract amendment #T01-0011-19-D
between Lewis County and The Salvation Army for $60,000 from January 1, 2018,
through June 30, 2020, is hereby approved and the Director of Public Health & Social
Services is authorized to sign the same

Purpose
The purpose of this amendment is to

It is mutually agreed therefore that the original contract#T01-0011-18 is hereby amended as
follows

Duration of the contract shall be extended to June 30th 2020

Increase budget amount by$60,000
for a total not to exceed $199,042 of all contracts and amendments as follows
