Call for bids for material purchase of precast concrete slab/girder bridge BEFORE THE BOARD OF COUNTY COMMISSIONERS LEWIS COUNTY, WASHINGTON IN THE MATTER OF: RESOLUTION NO. 20-083 CALL FOR BIDS (RE-BID AFTER ZERO BIDS RECEIVED FROM RESOLUTION 20-028) FOR THE MATERIAL PURCHASE OF A PRECAST CONCRETE SLAB/GIRDER BRIDGE (COUSINS RD. MP 3.15, CMP 1502) WHEREAS, Cousins Road crosses an unnamed tributary to Stearns Creek at mile post (MP) 3.15 where the existing culvert has partially failed. Replacement must meet current Washington Department of Fish and Wildlife (WDFW) fish passage guidelines, and a much larger structure is required. In order to prepare for replacement in the summer of 2020, competitive bids are being solicited from bridge manufacturers to supply and deliver standard precast concrete slab girder materials for this project; and WHEREAS, pursuant to Resolution 11-437, construction projects, including materials acquisition, exceeding a certain monetary threshold must be competitively bid; and WHEREAS, competitive bids will again be requested from concrete manufacturers to supply and deliver 45-foot long precast concrete slab/girders for bridge installation (by Lewis County) at Cousins Road MP 3.15 and other related work, all in accordance with the contract plans, contract provisions, and the standard specifications; and WHEREAS, pursuant to Resolution 18-116, the Board of County Commissioners (BOCC) adopted the 2018 Standard Specifications for Road, Bridge, and Municipal Construction and this proposal shall be performed by contract in accordance with these specifications. NOW THEREFORE BE IT RESOLVED that the Department of Public Works is instructed to re-issue a Call for Bids and Contract Specifications for the material purchase of a 45- foot long precast concrete slab/girder bridge (CMP 1502). Bids for this proposal shall be submitted to the Clerk of the BOCC (351 N.W. SCOPE Lewis County previously advertised and did not receive any bids for this material supply request. Therefore, items outside precast concrete manufacturer's typical construction operations have been deleted (guardrail design and waterproofing sealants). The purpose of this request for sealed bid is to supply and deliver 45-ft long precast concrete slab girders to be supported on a geosynthetic reinforced soil (GRS)foundation per the attached Plans and these specifications to provide a 28-ft wide travel surface. The "Superstructure —Cousins Rd MP 3.15 Bridge"shall be designed to support AASHTO HL-93 loading per the latest version of the WSDOT Bridge Design Manual. Bridge dead loads shall include the supplied superstructure, 0.23- ft HMA overlay, a future C.15-ft HMA overlay(not depicted), guardrail, extruded curb, and any other applicable permanent loads related to the superstructure. Precast units shall be connected using weld ties (or an approved equivalent) and grouted per the manufacturer's recommendation (on-site welding and grouting shall be by others). The outside slab girders shall include a drip edge and inserts (supplied by Lewis County) for side mounted steel posts. Ali guardrail and reducer section components to the bridge approaches shall supplied and installed by others, however, the manufacturer shall cast threaded inserts (supplied by Lewis County) for future guardrail attachment to the structure as indicated. The slab girder center joint and guardrail inserts at the outside precast units shall be designed to incorporate a 1.5% normal crown and 0.27% longitudinal slope per the Contract Plans. Two sets of superstructure plans stamped and certified by a Civil Engineer licensed in the State of Washington shall be provided to Lewis County within forty-five (45)working days of contract award. Plans shall include connection details, lifting details, assembly, and installation details. Contract Plans depict Guardrail with the slab girder superstructure depth at 18-inches and 30-feet wide to accommodate a 28-foot(minimum)finished roadway surface. Variations in superstructure width and/or depth due to various manufacturer's forms or construction methods will be accepted provided all listed requirements and the 28-ft minimum roadway surface is achieved with the proposed superstructure. 2. No fuel surcharges shall be charged. • Federal Tax Identification Number from IRS form W-9 is required on all invoices. • Bidders are to submit a copy of any and all warranties for the proposed product(s) with their response. A copy of the warranties shall also accompany products delivered. Page 1 of 8 • Slab Girder dimensions may vary due to manufacturer's existing forms and/or process. Manufacturer shall determine concrete thickness, dimensions/coniguration and design to achieve a 28-ft travel surface as depicted on Pages 5 through 8. Plans depict estimated thicknesses, dimensions, etc. The outside slab girders shall incorporate inserts groups of four threaded rods (two 11-inch long and two 7-inch long threaded rod supplied by Lewis County) at 37.5-inches on center for guardrail attachment by others. Threaded rod inserts shall be embedded to develop full strength with bolt groups starting/ending 0.75-feet from the ends (see Federal Highway Administration HSST— 1/B-264 for specific information regarding this side mounted weak post guardrail system). 3. SPECIFICATIONS The Contract shall be in accordance with the Washington State Department of Transportation 2018 Standard Specification for Road, Bridge, and Municipal Construction. All material components shall meet all requirements listed in the Washington State Department of Transportation 2018 Standard Specification for Road, Bridge, and Municipal Construction Section 6-02 Concrete Structures (including all applicable reference sections). 6-02.2 Materials Section 6-02.2 is supplemented with the following: 6-02.3 Construction Requirements 6-02.3(14) Finishing Concrete Surfaces Section 6-02.3(14) is supplemented with the following: The top surface of all slab girders shall have a light broom finish. 6-02.4 Measurement Section 6-02.4 is supplemented with the following: (August 2, 2010) ***Superstructure— Cousins Road MP 3.15 Bridge***contains the following approximate quantities of materials and work: PRESTRESSED CONC.
