APPROVE MEMORANDUM OF UNDERSTANDING
AMENDMENT A BETWEEN VALLEY VIEW HEALTH
CENTER AND LEWIS COUNTY TO PROVIDE
COVID-19 TESTING AND VACCINE
ADMINISTRATION
WHEREAS, the Board of County Commissioners (BOCC), Lewis County,
Washington, has reviewed Memorandum of Understanding Amendment #Valley
View Health Center-A between Lewis County and Valley View to provide COVID-19
testing and vaccine administration for Lewis County residents; and
WHEREAS, this amendment will be effective December 28, 2020, through June
30, 2021; and
WHEREAS, funding is provided from the State Department of Health (DOH) in the
amount of $100,000 added to the original amount of $400,000 for a maximum
total amount of $500,000; and
WHEREAS, it appears to be in the best public interest to authorize the execution
of said contract for Lewis County.
NOW THEREFORE BE IT RESOLVED that Memorandum of Understanding
Amendment #Valley View Health Center-A between Valley View Health Center and
Lewis County in the amount of $100,000 and extending duration through June 30,
2021, is hereby approved and the Director of Public Health & Social Services is
authorized to sign the same.

Description
Lewis County Public Health & Social Services receives funding from the Washington State
Department of Health to prepare, prevent and respond to the COVID-19 pandemic. The
purpose of this amendment is to extend the MOU duration, add to the Scope of Work and
add funds. Activities preformed through this MOU shall include COVID-19 testing and
vaccine administration for persons residing in Lewis County.
The MOU duration is extended through June 30, 2021.

through DOH expired December 31,2020 pursuant to state and federal guidelines.

Hourly rate for MA/DA $39.44
Hourly rate for reception $32.65
Hourly rate for management $90.21
Supplies as needed to perform vaccination per the contract Cost plus 10%
Vaccine coordination and referral $25 per patient
Total amendment funds added not to exceed $100,000
• Extend the duration to June 30, 2021
It is mutually agreed therefore that the original MOU Valley View Health Center is hereby
amended as described above and will go into effect December 28,2020.
All other terms and conditions of the original contract and any subsequent amendments hereto
remain in full force and effect. The parties signing below represent they have read and
understand this amendment, and have the authority to execute this amendment.
