APPROVE AMENDMENT BETWEEN LEWIS
COUNTY AND RELIABLE ENTERPRISES TO ADD
ADDITIONAL FUNDS THROUGH DDA IN THE
AMOUNT OF $5,960 AND REVISE PLANNED
SERVICE RATES
WHEREAS, the Board of County Commissioners (BOCC), Lewis County,
Washington, has reviewed Amendment #R01-0091-21-A with Reliable Enterprises
to increase planned rates and budget for direct services in accordance with the
increased rate lid put in place by the Developmental Disabilities Administration
(DDA) to individuals with developmental disabilities; and
WHEREAS, funding in the amount of $5,960 for a total maximum consideration of
$284,011 through the State DDA is provided for the period of July 1, 2020,
through June 30, 2021; and
WHEREAS, it appears to be in the best public interest to authorize the execution
of said contract.
NOW THEREFORE BE IT RESOLVED that Amendment #R01-0091-21-A between
Lewis County and Reliable Enterprises to provide services to individuals with
developmental disabilities in an additional amount of $5,960 for a total maximum
consideration of $284,011 and revise planned service rates from July 1, 2020,
through June 30, 2021, is hereby approved and the Director of Public Health &
Social Services is authorized to sign the same.

Description
The State Developmental Disabilities Administration contracts with Lewis County to
oversee the administration of grant money for services provided to individuals with
developmental disabilities. The purpose of this amendment is to increase the planned
rates for direct services in accordance with the increased rate lid put in place by the
Developmental Disabilities Administration as of March 1, 2021 . In addition, this
amendment will add funds to cover the rate increase for direct services.

Purpose
The purpose of this amendment is to:
• Make revisions to Exhibit B,Budget Detail and planned service rates
• Increase total budget amount by$5,960.00
It is mutually agreed therefore that the original Contract#R01-20 is hereby amended as follows:
1. The first paragraph of Exhibit B is amended to say"not to exceed 284,011.00" instead
of"not to exceed $278,051.00."
2. Item 1, Budget Detail is replaced in full with the following:
Individual Supported Employment(ISE) Not to exceed hourly rate of$105
Community Inclusion(CI) Not to exceed hourly rate of$49
Time-limited COVID activities related to Not to exceed hourly rate of$75
ISE and CI
Child Development Services(CDS) $52,051
Family Support Network $25,000.00
Home Nutrition Services $12,000.00
SPARC $25,000.00
Total Consideration $284,011.00
ISE, CI and CDS Services must be provided pursuant to a County Service Authorization.
3. The Direct Service Rates table in Item 2,Compensation is deleted.
This amendment conforms to federal and state reimbursement rule changes retroactively
effective for services rendered on or after March 1, 2021. Therefore, services rendered on or
after March 1, 2021 shall be billed and reimbursed according to this amendment. The County
Professional Services Contact#RO1-0091-21-A
and Contractor agree that no previously submitted billing requires correction or supplementation
as a result of this retroactive amendment because March services have not yet been billed.
All other terms and conditions of the original contract and any subsequent amendments hereto
remain in full force and effect. The parties signing below represent they have read and
understand this amendment, and have the authority to execute this amendment.
