Approve amendment with Equity Institute to remove funds in the amount of $90,800 from Outreach and Prevention Case Management. BEFORE THE BOARD OF COUNTY COMMISSIONERS LEWIS COUNTY, WASHINGTON IN THE MATTER OF: RESOLUTION NO. 21-184 APPROVE AMENDMENT BETWEEN LEWIS COUNTY AND EQUITY INSTITUTE TO REMOVE FUNDS IN THE AMOUNT OF $90,800 FROM OUTREACH AND PREVENTION CASE MANAGEMENT WHEREAS, the Board of County Commissioners (BOCC), Lewis County, Washington, has reviewed Amendment #EquityESG-CV-B with Equity Institute to prevent, prepare for and respond to the COVID-19 pandemic among individuals and families who are homeless or receiving homeless assistance; and WHEREAS, funding in the amount of $90,800 will be removed from Outreach and Prevention Case Management for a total maximum consideration of $104,200 for the period of August 1, 2020, through June 30, 2022; and WHEREAS, statement of Work and Reporting Requirements need to be updated; and WHEREAS, it appears to be in the best public interest to authorize the execution of said contract. NOW THEREFORE BE IT RESOLVED that Amendment #EquityESG-CV-B between Lewis County and Equity Institute to prevent, prepare for and respond to the COVID-19 pandemic removes an amount of $90,800 for a total maximum consideration of $104,200 from August 1, 2020, through June 30, 2022, is hereby approved and the Director of Public Health & Social Services is authorized to sign on behalf of Lewis County. The Act includes Emergency Solutions Grant stimulus funding (ESG-CV) to be used to prevent, prepare for, and respond to the coronavirus pandemic (COVID-19) among individuals and families who are homeless or receiving homeless assistance. The funds also support additional homeless assistance and homelessness prevention activities to mitigate the impacts of COVID-19. The purpose of this amendment is to update EXHIBIT A: STATEMENT OF WORK AND REPORTING REQUIREMENTS, Task 1 to include: `targeted vaccine ambassador outreach' verbiage; Remove Task 2 Prevention Case Management; and update EXHIBIT B: BUDGET, INVOICING, AND CONTRACTOR PAYMENT to remove $90,800 from current Equity Institute budget for Prevention Case Management and Outreach - for a new total contract budget of$104,200. The need to amend is driven by the need to remove Prevention Case Management as a task assigned under this MOU. • Change EXHIBIT B: BUDGET, INVOICING, AND CONTRACTOR PAYMENT to remove$75,000 from `Prevention Case Management', and$15,800 from `Outreach' budget as listed in this MOU. The need to amend is driven by removal of`Statement of Work and Reporting Requirements,Task 2—Prevention Case Management' under this MOU; and update to budget allocated to `Outreach' under this MOU. MOU,EquityESG-CV-B,EQUITY INSTITUTE ESG OUTREACH It is mutually agreed therefore that the original MEMORANDUM OF UNDERSTANDING, MOU#EquityESG-CV, MOU Equity Institute ESG Outreach is hereby amended as follows: Exhibit A: Statement of Work and Reporting Requirements The purpose of this Statement of Work is to detail the work to be performed by Equity Institute and the methods and content for reporting progress by Equity Institute in fulfilling all duties encompassed in this MOU. The term of this MOU shall commence on Decemberl,2020 and terminate on June 30,2022. Equity Institute shall administer funds awarded hereunder, to support a variety of activities, including operations of time-limited housing units, rental assistance, and data collection and reporting, coordinated assessments, legislatively established priorities, and requirements for a local homeless plan.
