COUNTY AND RELIABLE ENTERPRISES TO ADD
ADDITIONAL FUNDS IN THE AMOUNT OF $75,000
AND UPDATE REQUIREMENTS TO BE USED FOR
PEOPLE WHO ARE HOMELESS OR RECEIVING
HOMELESS ASSISTANCE
WHEREAS, the Board of County Commissioners (BOCC), Lewis County,
Washington, has reviewed Amendment #R01-0104-20-B with Reliable Enterprises
to prevent, prepare for and respond to the COVID-19 pandemic among individuals
and families who are homeless or receiving homeless assistance; and
WHEREAS, additional funding in the amount of $75,000 for a total maximum
consideration of $131,112 through the Emergency Solutions Grant stimulus
funding (ESG-CV) is provided for the period of August 1, 2020, through June 30,
2022; and
WHEREAS, it appears to be in the best public interest to authorize the execution
of said contract.
NOW THEREFORE BE IT RESOLVED that Amendment #R01-0104-20-B between
Lewis County and Reliable Enterprises to prevent, prepare for, and respond to the
COVID-19 pandemic in an additional amount of $75,000 for a total maximum
consideration of $131,112 from August 1, 2020, through June 30, 2022, is hereby
approved and the Director of Public Health & Social Services is authorized to sign
the same.

Description
The Coronavirus Aid, Relief, and Economic Security (CARES) Act was authorized on
March 27, 2020. The Act includes Emergency Solutions Grant stimulus funding (ESG-CV)
to be used to prevent, prepare for, and respond to the coronavirus pandemic (COVID-19)
among individuals and families who are homeless or receiving homeless assistance. The
funds also support additional homeless assistance and homelessness prevention activities
to mitigate the impacts of COVID-19.
The purpose of this amendment is to update EXHIBIT A: STATEMENT OF WORK AND
REPORTING REQUIREMENTS, Task 1 to include: `targeted vaccine ambassador
outreach' verbiage; and update EXHIBIT B: BUDGET, INVOICING, AND CONTRACTOR
PAYMENT to add $75,000 to current Reliable Enterprises budget for Outreach - for a total
contract budget of $131,112.

Purpose
The purpose of this amendment is to:

part of the Lewis County 5-year Housing and Homeless Strategic plan and the COVID-
19 pandemic,provide vaccine ambassador outreach services to marginalized populations
in Lewis County'; and Task 1, DELIVERABLES/OUTCOMES to include `targeted
vaccine ambassador outreach',
The need to amend is driven by the need to update verbiage of Task/Activity/Description
and Deliverables/Outcomes.
• Change EXHIBIT B: BUDGET, INVOICING,AND CONTRACTOR PAYMENT to
remove$9,612 from Prevention Other, and add$84,612 to current budget for Outreach.
The need to amend is driven by additional funds added to budget.
It is mutually agreed therefore that the original PROFESSIONAL SERVICES CONTRACT,
RELIABLE_ESG-CV is hereby amended as follows:

The purpose of this Statement of Work is to detail the work to be performed by the Contractor
and the methods and content for reporting progress by the Contractor in fulfilling all duties
encompassed in this contract.
The term of this contract shall commence on August 1,2020 and terminate on September 30,
2022.
The Contractor shall administer funds awarded hereunder, to support a variety of activities,
including operations of time-limited housing units,rental assistance, and data collection and
reporting, coordinated assessments,legislatively established priorities, and requirements for a
