MOU Amendment B with Valley View Health Center to extend agreement providing COVID-19 testing BEFORE THE BOARD OF COUNTY COMMISSIONERS LEWIS COUNTY, WASHINGTON IN THE MATTER OF: RESOLUTION NO. 21-329 APPROVE MEMORANDUM OF UNDERSTANDING AMENDMENT B BETWEEN VALLEY VIEW HEALTH CENTER AND LEWIS COUNTY TO EXTEND AGREEMENT PROVIDING COVID-19 TESTING WHEREAS, the Lewis County Board of County Commissioners has reviewed Memorandum of Understanding #ValleyView_COVID-19_2021-B between Lewis County and Valley View Health Center to extend agreement for 90 days to provide testing strike teams to serve at Lewis County's request for Lewis County residents; and WHEREAS, this amendment will be effective January 21, 2021, through October 18, 2021; and WHEREAS, task 2, administration of COVID-19 vaccines to residents of Lewis County, is removed as vaccine services are now being performed by Bird's Eye Medical; and WHEREAS, there is no change in the funding amount, which was originally $300,000; and WHEREAS, it appears to be in the best public interest to authorize the execution of said contract for Lewis County. NOW THEREFORE BE IT RESOLVED that Memorandum of Understanding Amendment #ValleyView_COVID-19_2021-B between Valley View Health Center and Lewis County, which extends duration through October 18, 2021, for testing and removes administration of vaccine, is hereby approved and the Director of Public Health & Social Services or his designee is authorized to sign the same. The MOU is for Valley View Health Center to provide testing strike teams to serve at Lewis County's request. Task 2 (administration of COVID-19 vaccines to residents of Lewis County) is being removed — as the vaccine services are now being performed by Birds Eye Medical. The contract will be further amended to extend the duration beyond October 18, 2021 if services are still required for the health and safety of the residents of Lewis County, and funding is still available. The term of this MOU shall be hereby amended to commence on January 21, 2021 and terminate on October 18, 2021. • Remove Task Number 2 from Exhibit A: Statement of Work and Reporting Requirements. Task 2 (Provide COVID-19 vaccine consultation, coordination, and administration of the COVID-19 vaccine to Lewis County Residents) is hereby removed from MOU# ValleyView_COVID-19_2021. Documentation of hours in support of Task 1, including what staff(scribe/LPN/RN etc.) has provided the service with date, location, and time in quarter hour increments will still be required when submitting invoices for services rendered in support of MOU# ValleyView_COVID-19_202 1.
