APPROVE AMENDMENT E TO MASS VACCINATION
AGREEMENT BETWEEN LEWIS COUNTY AND
BIRD'S EYE MEDICAL FOR COVID-19
VACCINATIONS
WHEREAS, the BOCC has reviewed Amendment
#Birdseyemedical_MassVacCOVID-19-E with Bird's Eye Medical to provide
COVID-19 vaccine consultation, coordination and administration; and
WHEREAS, this amendment extends the term of the agreement through April
16, 2022; and
WHEREAS, funding in the amount of $150,000 is added for a total amount of
$1,300,000; and
WHEREAS, it appears to be in the best public interest to authorize the execution
of said contract.
NOW THEREFORE BE IT RESOLVED that Amendment
#Birdseyemedical MassVacCOVID-19-E between Lewis County and Bird's Eye
Medical to extend the agreement through April 16, 2022, and add funds to
provide COVID-19 vaccine consultation, coordination, and administration in the
amount of $150,000 for a total of $1,300,000 is hereby approved, and the
Director of Public Health & Social Services is authorized to sign the same.

The need to amend is driven by continued need for services through January 16th, 2022 and

2021 and terminate on April 16th,2022.
