WHEREAS, the Consolidated Contract requires further amendment; AND
WHEREAS, Amendment #1 adds Statements of Work for COVID -19 Mass Vaccination,
Foundational Public Health Services (FPHS), Maternal & Child Health Block Grant, Office of
Drinking Water Group A & B Programs, Office of Immunization COVID -19 Vaccine,
Supplemental Nutrition Assistance Program - Education, and WIC Nutrition Program; AND
WHEREAS, this amendment increases the Contract by $2,425,877 which shows a
revised maximum consideration of $2,425,877; AND
WHEREAS, this amendment is effective January 1, 2022; AND
WHEREAS, it appears to be in the best public interest to authorize the execution of said
contract for Lewis County;
NOW, THEREFORE BE IT RESOLVED that Amendment #1 to the Consolidated
Contract #CLH31017, between Lewis County and DOH, adds Statements of Work which
provides for an increase of $2,425,877 for a total maximum of $2,425,877, is hereby
approved and the Director of Public Health & Social Services is authorized to sign the same.

PURPOSE OF CHANGE: To amend this contract between the DEPARTMENT OF HEALTH hereinafter referred to as "DOH ", and
LEWIS COUNTY PUBLIC HEALTH & SOCIAL SERVICES DEPARTMENT, a Local Health Jurisdiction, hereinafter referred to
as "LHJ ", pursuant to the Modifications /Waivers clause, and to make necessary changes within the scope of this contract and any
subsequent amendments thereto.
IT IS MUTUALLY AGREED: That the contract is hereby amended as follows:

reference, are amended as follows:
