Appointing two (2) members to the Lewis County Public )
Health & Social Services (PUSS) Advisory Board ) Resolution No. 021323.1
WHEREAS, the Lewis County Public Health and Social Services (PHSS) Advisory Board was
established by the Lewis County Board of County Commissioners (BOCC) via Resolution 17 -153
and later transferred authority to the Lewis County Board of Health (BOH) via BOCC Resolution
22 -085 and BOH Resolution 041822.2; and
WHEREAS, this Advisory Board is comprised of eleven members and was established as a voice
of the community and to make recommendations to the BOH; and
WHEREAS, Elizabeth Rohr and Rebekah Miner's terms ended December 31, 2022 creating two
vacancies, one for a Medical Provider position and an At -Large position; and
WHEREAS, Dr. David Ellis has expressed interest and willingness to serve and represent the
vacant Medical Provider position and Deb Mizner has expressed interest and willingness to serve
and represent the vacant At -Large position; and
WHEREAS, the BOH has reviewed Dr. David Ellis and Deb Mizner's applications and wish to
appoint Dr. David Ellis and Deb Mizner to begin serving three (3) year terms through December
31, 2025; and
NOW, THEREFORE BE IT RESOLVED the Lewis County BOH hereby appoints Dr. David
Ellis to represent the vacant Medical Provider position and Deb Mizner to represent the vacant At-
Large position on the Lewis County PHSS Advisory Board through December 31, 2025.
